WELCOME

Dr. Douglas Daly and staff welcome you. We designed this brochure to give you information about our office and answer some questions we are routinely asked. We hope it proves helpful now, and in the future, as a reference.

OUR PRACTICE

We are dedicated to providing the highest quality medical care and treatment for persons experiencing varying degrees of infertility. As a specialist in Reproductive Endocrinology, Dr. Daly has advanced skills in helping couples achieve their dream of having a natural child and in other areas of gynecology such as treatment of endometriosis, uterine fibroids, and problems associated with hormonal variations.

At Grand Rapids Fertility & IVF, we can provide the latest in Assisted Reproductive Technology (ART) to diagnose and treat infertile or sub-fertile couples. We also have a special interest in male infertility and offer a range of services in this area. We work closely with Spectrum Health East in providing the additional staffing and equipment required for the advanced technology associated with ART.

OFFICE PRACTICES

Office Hours

Our office hours are 8:00 am to 4:00 pm. Monday through Friday except holidays. Because so much of our work is dependent on the menstrual cycle of the patient, we also offer limited services on Saturday or Sunday at Dr. Daly's discretion.

Telephone

Our telephones are answered from 8:30 am to 4:30 pm, Monday through Friday, except holidays. For an appointment, call (616) 774-2030 or 1-800-695-5941. Routine questions about your care or treatment are best answered during office hours when your medical records are available. If Dr. Daly is with a patient when you call, the nurse will confer with the doctor and relay instructions to you or have him return your call. Generally, our answering service will take your calls for 45 minutes during the lunch hour.

Appointments

Please make every effort to keep your scheduled appointment. If you cannot, 24 hour notice would be greatly appreciated-- please call and cancel. We make every effort to stay on time, but unavoidable circumstances will occasionally set us back. Please be assured that we will take as much care with you when it is your turn.

First Consultation

Treatment for infertility is a joint undertaking by all parties, therefore we require that both you AND your spouse/ partner be present for this appointment. This hour-long appointment gives Dr. Daly time to review your medical and personal history, get acquainted with you, develop a diagnostic plan and present your available options for treatment. It is very important that we have medical records pertaining to gynecologic/obstetric/infertility care from other physicians, especially the operative reports from any gynecological/pelvic surgery. These records should be obtained from the doctor or hospital where treatment took place PRIOR to your appointment, If your spouse/partner has had any related medical work, his records are valuable too, and should also be sent to us prior to your appointment.

If you are seeking Dr. Daly’s help for something other than infertility, such as endometriosis or uterine fibroids, it is still important that you obtain your medical records. Your spouse/partner is not required to accompany you on this first visit, but their support is welcome. Your initial consultation is scheduled for approximately one hour.

Emergency/After Hours

If you have an emergency, please call our office and the call will be taken by our answering service. They will contact Dr. Daly or the physician on-call, who will call you. In the rare instance that you are not able to reach our answering service, use Spectrum Health East Emergency as a backup.

FINANCIAL POLICIES

Insurance coverage for infertility and related services varies greatly from company to company and policy to policy. We will offer you any information we have available about current reimbursement trends to help you obtain the maximum benefit from your insurance coverage. Many insurance companies/employers, however, do not offer coverage for many of our services. Dr. Daly is a referral specialist for a variety of HMOs, PPO, and managed care programs. We will bill all covered services to these companies for you, but you will be responsible for any co-pays, deductibles, or non-covered services that are billed to you. We do not offer a regular payment schedule; you are asked to remit any balance due in full. Our cashier will let you know if your services will be submitted by us to your insurance company for payment.

Check out -- You are asked to stop at the discharge desk and pay for each visit in full at the time of service. For your convenience, we accept MasterCard and Visa.

Medical Insurance Payments --You will routinely be given an encounter form for each date of service. This form lists your diagnosis, service(s) performed, and the fees. Simply fill out the insurance claim form provided by your insurance company and attach our encounter form. It will serve as the "Attending Physician Statement." Reimbursement is strictly between you and your insurance company and will be made directly to you.

Non-Covered Services -- If an option presented to you is not covered by your insurance company, and is an elective procedure, you will be required to make advanced payment for the service. Please contact any of the business office personnel for information and fees.

Fees -- We will be happy to discuss all fees and discuss any questions you may have regarding our services.

Managed Care (HMO) Patients -- It is ultimately YOUR responsibility to verify that we have the proper authorization on file and it is CURRENT for your care in our practice. Please contact your Primary Care Physician for referral and continuation of care. NEVER assume that your doctor has referred indefinitely for fertility treatment. Also, confirm that you have proper authorization for any special procedure or surgery scheduled through our office.

LOCATION

Grand Rapids Fertility & IVF is located in Suite 315 at the Professional Office Building (POB) on the grounds of Spectrum Health East. Public parking is available in the garage attached to the POB.

If coming from North/Muskegon: Take I-96 east & exit onto East Beltline going south, take Fulton St. to the west (right) and follow the blue and white "H" signs, going left on Robinson Rd. Continue to follow the blue and white "H" signs to Plymouth Ave. (approx. 1 mile) turn left on Plymouth Ave. and go one block to Wealthy St., turn left again and proceed to the Spectrum East (Blodgett) Hospital entrance. The hospital is on the right and we are located on the left in the Professional Building, Suite 315.

If coming from the West/Holland: Take I-96 exit onto the East Beltline going south, take Fulton St. to the west (right) and follow the blue and white "H" signs to Plymouth Ave., (approx. 1 mile) turn left onto Plymouth Ave. and go one block to Wealthy St., turn left again and proceed to the Spectrum East (Blodgett) Hospital entrance. If coming from the South:

Take Burton exit off of US 131 east to Plymouth Ave., (approx. 3 miles), turn left and go north onto Plymouth Ave. (approx. 2 miles), to Wealthy St. and turn right, proceed through the Spectrum East (Blodgett) Hospital entrance. The hospital is on the right and we are located on the left in the Professional Building, Suite 315.

DOUGLAS C. DALY, M.D.

Douglas C. Daly, M.D., is a graduate of the University of Connecticut School of Medicine and completed his Reproductive Endocrinology Fellowship in 1982 at the same institution.  Board Certified in Obstetrics, Gynecology and Reproductive Endocrinology, Dr. Daly is a Fellow of the American College of Obstetrics and Gynecology (F.A.C.O.G.) and a member of the American Society of Reproductive Medicine (ASRM).  In 1991 he assumed leadership of the Assisted Reproductive Program at Blodgett Memorial Medical Center, now known as Spectrum Health East. This program maintains membership in the Society for Assisted Reproductive Technology (S.A.R.T.) and abides by the regulations and guidelines of this organization.  Dr. Daly is also a Clinical Associate Professor in Obstetrics and Gynecology at Michigan State University (MSU). He is an active staff member of Spectrum Health East and St. Mary's Hospital.
