INFERTILITY
Infertility is traditionally
defined as one year of intercourse without conception. In the
absence of a condition or symptom that suggests a reason for infertility
it is appropriate for couples/women less than 34 years of age to wait for
10 - 12 cycles before becoming concerned. However, if any of the
following are present consultation should be sought earlier:

- Cycles are irregular or absent
- A history of sexually transmitted
infection
- Significant endometriosis
- Male urologic or
genital issues
- Either partner has significant
general health problems
(i.e. diabetes or kidney disease)

| In women 34 and
older, failure to conceive in 6-8 months is not uncommon but because of
age and time issues consultation may be sought earlier than in
younger women.
Female
Infertility
50% of
infertility is primarily caused by a problem in the woman. Another
25% is a couples issue involving both the male and female. The three
primary problems found in women are: anovulation, endometriosis and
tubal disease. Evaluation should include testing in all three areas.
When no male or female problem is found this is known as
"unexplained" infertility.

Male Infertility
25% of infertile couples
will have a male factor as the primary problem. Another 25% will
have a male factor as part of the overall problem. Therefore,
initial consults should be done as a couple and a semen analysis should
be one of the first tests performed. Differentiating a fertile semen from sub fertile or infertile semen is difficult. We
recommend that semen analysis for fertility be done in an IVF/fertility
facility rather than a general laboratory.
Before a couple considers a vas reversal, it is important for the wife
to have a basic evaluation. If the vas ligation is more than 5
years old, the couple may wish to do a testicular biopsy and an IVF-ICSI
approach. In selected situations, even couples with a ligation
less than 5 years in duration may benefit from this approach. More
